Magento's password reset page is blank after upgrade

A problem I've run into twice now. This is a result of theme differences between older and newer version of Magento - the most recent case involving an upgrade from CE 1.5.1.0 to 1.9.0.1. For initial upgrade, we kept the 1.5.1.0 theme. Apparently newer versions of Magento include a segment of code in the customer.xml that is not featured in older versions.

// Navigate to app/design/frontend/your_package/your_theme/layout/customer.xml and 
// look for the customer_account_forgotpassword section that ends with
// "</customer_account_forgotpassword>" and add the following:
